Once in a blue moon you can see someone going topless at Spa 88 on Fulton Street or at the Tenth Street Baths, but not very often, and full nudity is not tolerated. Sorry not to have better news, but Manhattan still doesn't have any nude-friendly spas. (The exeception, of course, is single-sex spas, where you generally can go naked all you want. But that's presumably not what you're looking for either.)
