No change, sadly, since the last time this question was asked, or the ten times before that. Once in a blue moon you can see someone going topless at Spa 88 on Fulton Street or at the Tenth Street Baths, but not very often, and full nudity is not tolerated. If you're spotted going naked at either place, you'll be told to cover up at best, thrown out at worst. Does that mean you can't go naked? No, but you have to do it when no one is there to see you and be prepared to cover up the instant someone you don't know walks through the door. It's tense and stressful, the exact opposite of the sort of relaxing, fun environment you're probably looking for.
Sorry not to have better news, but Manhattan still doesn't have any nude-friendly spas. (The exeception, of course, is single-sex spas, where you generally can go naked all you want. But that's presumably not what you're looking for either.)
